License Form with signature

I just need to create a license form for my customers where they fill in their name and product number (product from our website).
Next they will be able to download the form as PDF file.
The license.pdf will contain their name, product number, date and my signature.Is this possible with your plugins?

The plugin will generate PDFs with your Gravity Forms data, yes. There are four layouts included out of the box. If you’d like a custom layout, you can design it yourself using PHP/HTML/CSS, or have our team create the template for you.
