Leads not going to Salesforce

We have installed the plugin, created a very basic minimal form. No fields required. Once filled out the the form processes and gives me back the message:
Failed to connect to Salesforce.comWhen we check Salesforce we are not getting the lead.
We checked to be sure we input our Salesforce oid correctly.
We went into salesforce, created a web to lead form, processed it in a browser and that form lead went into Salesforce.
