Leadin and WooCommerce Checkout Manager – many "Not checked" fields in form

Hello,
I have problem when i use WooCommerce Checkout Manager & Leadin.
When leadin send me mail about new contact, Leadin also send me info about all “non checked” fields and this is a problem.
How to hide this non checked fields ?Screen:
