Lead Referral for Checkout Page? Vendor Manager?

Hi Guys,

I haven’t had much finding a plugin that could handle this and integrate with woocommerce.

So I’m a consultant that offers free industry reports to my customers. When a customer decides they’d like to view the information/download a report, I’d like to offer them the ability to opt-in to a vendor referral program. For instance, if a customer is interested in getting some quotes on production equipment, they could fill out a short survey describing what equipment they’re interested in and I could put that survey form on either my woocommerce checkout page or on my product download page?

This next feature would be nice to have, but isn’t a must. I’d like to have my vendors be able to register for an account. When a client fills out a survey that matches the ‘equipment’ they sell, it would send them the lead and charge them a flat rate for the referral.

Are there any plugins that would allow something like that to be accomplished, or would I have to design something like that myself?
