Lead generation form in the header

Is it possible to include a lead generation form – 2 fields needed: name and email – in the Freddo header as in this LaunchKit theme: https://launchkit.tommusdemos.wpengine.com/home-multi-purpose/
I would also like to include the same format the end of the homepage but I find it really hard to customize.Many thanks,
