I have found the issue that is creating problem there. Each line brake is a separate line. For some reason it is rendered as element :

here is the solution for your issue. Go to plugin settings > customize page. There you will see field custom style. Add this there and save the page :

.ea-bootstrap { white-space: nowrap; }

we installed and setup the connector. However. the page is not redirected to woocommerce after user submits the appointment form.
