labels above inputs for submission form on “request a quote” page not displaying

In “request a quote” plugin by Yith, when I try to complete submission form it does not display the label of “name” and “email” above the text box on the “request a quote” submission form page. It also does not display text for the “register” checkbox label. So, the input space to enter name/email is there but there is no heading.

I had this problem after selecting the “enable customers to register” option in form settings. How do I get the “request for quote” form to display properly? I try turning this option off and reinstalling plugin but nothing works.
