Just not working for me

This looked like the perfect solution for what I am trying to achieve with gravity forms. However, after installing, setting up, and inserting a shortcode where i want to see submissions, i just get a blank space. I see this hasnt been updates in over a year, can anyone confirm that this is still working on the latest wordpress and gravity form versions?
