Join Email List Feature

Does anyone know a way of putting a simple textfield that accepts an email address with a submit button in the footer. I’ve tried the online form creators but can’t seem to figure out how to direct the user to a template page that displays a “thank you” message. I’ve tried putting the get_header and get_footer as well as include() with TEMPLATEPATH as well as just providing the full absolute address to each file.
