Jetpack Contact Form – Consent Needed

Hello!
I’m using a Jetpack Contact form to obtain subscribers name and email address. In order to import this information into creative mail, I need to have their “consent” to provide this information. However, when using the classic editor in Word Press, I cannot find language or a box to click to add to this area so the consent verification can be stored. I am still testing now, but can see as a test subscriber that the feedback comes back with this information stored (below).
[email_marketing_consent] =>
If consent was given, it should read like this:
[4_Consent] =>
[email_marketing_consent] =>Ideally I would like the user to check a box acknowledging their consent. Please help!

I don’t see that when I submitted the form:
The link you sent just goes to your form (rather than results). Would you send us a screenshot of what you’re seeing, and let us know where (in WP Admin somewhere, or after form submission on the front end) you see the issue?
