Jet Pack contact form distorted after latest update

I am building my first wordpress site and just downloaded the latest updates for the theme (twentytwelve) and jetpack. My contact page, which looked perfect before the update, is now distorted…the fields shrunk and do not line up.

Is there a fix? I am not a coder.

Does anyone have a better plug in for a simple contact form?
