Java date picker populating wrong

I am working on my localhost so I don’t have an example of the issue but can upload my website and send a link if needed… I’m just hoping there is something I am missing with my form setup. I have enabled JavaScript Date Picker and am trying to use it on my form, however, it is coming up without a background making it very difficult to see and seems buggy besides, because the top line showing previous month, next month, etc… is also mid-aligned.
