You could add custom fields to your donation form to request this information from your donors. Their entries would then be collected in the Transaction Details.

You can do this in two different ways:
1) If you are comfortable with WordPress coding and development you can do this manually based on our documentation:

2) You can purchase our Form Field Manager Add-on which gives you a simple UI directly in your Form Edit screen to add custom fields.
