It was work perfectly, now it’s not

Hello, last month I installed this plugin and work perfectly. But for some reason I have to reinstall my blog, and when I instal this plugin again, it’s not working. The page won’t show the map. For your information, I have to deactivated all plugins, but still not working properly. Sometime it show the map, but most of it, it’s not
