    Hi,

    Problem has been solved. It was related with our CDN that serves JavaScript files on WordPress plugin.

    I have the same issue as Graham Jones did. Click the icon, a list of my forms comes up, I click on the form I want and nothing happens.

    Can you help me out with it?

    I am now closing this 11 month old topic as it references an older version of WordPress.
