issues with caldera form

    Try make a new page with the short_code inside, and see if this new page can display your form properly or not, you can try to change the slug of your element too and please avoid using one letter, it would be great if your slug more than 3 letter and consists of the underscore, example:
    – %Address%
    – %Full_Name%
    – %Slug_Name%

    i have tried to do what you suggest but the issue still there.
    Here is another issue: that i attach
    some strange caracters are displayed before the fields
