Issue with Multiple Checkboxe Qustion with identical choices

  • Hi,

    We are trying to create a feedback form on our website – and because of this we need to ask several different questions, but allow the same answer (Like a scale of 1-5 to be rated for each topic).

    Even though each checkbox and question is given a separate identifier – the forms still think the subsequent checkboxes are for the first question – meaning the forms can’t be used!

    Any help would be greatly appreciated.
