I was able to confirm that a default install with just Gravity Forms and Members worked as expected.
After a little digging it we determined that a 3rd party had modified the admin user role and removed access to Gravity forms and a few other sections of the backend.
So the plugin was working as expected just with a non-standard configuration in place for the administrator role.
With that in mind, the issue presents itself in a confusing manner and displays other plugin’s menu items under the restricted section. For example gravity forms displayed menu items for Enable Media Replace “Replace Media” and The Events Calendar “Freemius debug” https://share.getcloudapp.com/12uKPkLd
