Issue With Form Appearing

Hello,

Can you please let me know what the issue with the following page is please. The plugin was working fine and then it stopped appearing: https://clhpainting.misgoodbuildsite.info/

It is attached to button as a Modal. If you visit any of the other pages on the site you will see the form works on one button but if it has two buttons on the same page then it will only appear on one, any do the same thing of not appearing on the other button). Is this an issue with the Plugin or the Theme itself that can be sorted out since both are new versions.
