Issue with Contact Form 7 Submit

I have set up a new contact form and when I’m testing I get the message:

“YOU MUST ACCEPT THE TERMS AND CONDITIONS BEFORE SENDING YOUR MESSAGE.”

I have tried adding a acceptance option, but still receive the same message. I’m at a loss as to why this would happen. Any suggestions?

Unfortunately the page in question is in staging mode pand password protected so I can’t share the link. Maybe the issue is with Stagecoach (the staging option I’m using?)
