Issue when utilizing multiple lightboxes.

Hi There, I am using the lightbox plus to add multiple lightbox lead captures to my websites page.
Functionality:
The user clicks on download report and a form pops up.
The user fills out the form, clicks accept button and then clicks submit
The page refreshes and all downloads are available to them at that time.Current State:
If clicking on the first lightbox on the page “lbp-inline-link-1 cboxElement” the form pops up and the user is able to complete the form successfully. The form also successfully executes as the database is updated with the correct email and name as well as file downloaded.
If selecting any of the others “lbp-inline-link-2 cboxElement” for example the form pops up but the submit button is not clickable regardless if form is completed or not.
If I change all lightboxes to the first “lbp-inline-link-1 cboxElement” lightbox on the page. The form fills out correctly again however the first download on the page (the first link using the lightbox on the page) loads regardless of which download you select.

Any insight on this topic would be gratefully appreciated.
Please advise as to if you have any insight on how to rectify this issue as it seems to be related to utilizing more then one lightbox in a single page.here is the page in question with the issue.
