Issue trying to use the Display Prayer Requests shortcode

whenever I add this shortcode to the site [wp-prayer-engine] the page crashes and won’t load till I remove it. The praise report does allow the header to load but the body is blank. (Just FYI as I’m only trying to allow prayer request and display them and am not using it, but it may help understand the issue) When I put in the [wp-prayer-engine form] onto a page the page says cheating….
