issue displaying the form

I am using Sahifa theme.

I am using a theme shortcode (tabs) and adding the contact form shortcode within the theme shortcode.

The problem is that under each tab all the 3 contact forms are shown. I want only 1 form shown up per tab.

This is the shortcode on my page :
