Is there any plugin like this? Writer fills form – results on your site

Hello everyone,
I’m looking for a specific plugin that basically can do the following things:
1: An administrator should be able to customize a form and choose the fieldnames and fieldtypes and stuff. The form doesn’t have to be really advanced.
3: A blogwriter (non-admin) can login in his panel and could fill this form with a new entry/input.
4: The blogwriter selects a category where the form entry would fit in.
5: After submitting it, the content of the form will be visible on the homepage of the wordpress-site in a widget with the “10 latest items list” which contains the title of the 10 latest form entries of any of the available category’s. (widgets for every custom category would be welcome too, but maybe thats too specific. So thats not a must).
6: Users are able to open a page on your wordpress-site with all the form categories on it. When the user clicks on a category there should open a new page with a list with all the title’s of the form entries in that specific category.
7: When a user clicks on the title of a single entry, it should open it’s own page and show the full content of the form entry that the blogwriter had written in his panel.
In my opinion something like this could maybe exist. I’ve searched for ages and used a lot of keywords in Google to search for it too. I just didn’t find anything yet that comes close to my wishes.
I would really appreciate it if any of you could help me find a suitable plugin that I could use. All help is welcome and appreciate. Thanks in advance!
