IS there a way to add a Expire Headres to wordpress-file-monitor.php?

Had to uninstall the plugin again because of this, had already forgotten that it’s there on all pages after activating it ??

I see no reason why this kind of security script is
a) visible to all
b) loaded on the front end.

It takes a good 500ms to load the thing for no good reason, plus it can’t be optimal for security to have it exposed.

Is there a reason not to use wp-cron to run it with a timer in the back-end?

I’m no longer publicly supporting the plugin, it’s fulfilling its need for me. But another developer is more then welcome to fork it and continue work on it if they would like. I might release an update in the future, but for now, it’s doing what it’s supposed to.

It loads as a CSS file so that it won’t delay your perceived page load time when it’s scheduled time to run comes around.

I believe this was answered already in one of the threads you linked to.

Its a complete re-write of WordPress File Monitor.

It doesnt use a stylesheet to trigger scans but uses the cron built into wordpress, or if you prefer you can run it via an external cron.
