IP detection on forms shows same IP on all form entries

For instance all entries on this form https://nametagpros.com/free-samples/ and all the forms build with this plugin are showing 162.144.52.217 as the IP address. This is the IP of the site not the user who filled out the forms. How can it be set up to capture the users IP so I can block malicious entries?
