IP Blacklist not happening as expected

As the sole admin of our site I keep very tight control of logins. I have turned up the blacklist settings to have the system automatically blacklist an IP after 2 lockouts in 90 days.
Or so I thought. Today I got my 3rd email lockout notice for the same IP in the past 24 hours.
When I check the plugin logs I clearly see over a dozen lockouts to that IP in the past day. So my question is why isn’t that IP automatically added to my blacklist and prevented from just waiting out the lockouts? Is there something else I need to configure or should the plugin have blacklisted that IP permanently?
Oh yes, I did enable the “Allow iThemes Security to write to wp-config.php and .htaccess.” setting so that the .htaccess file can be updated if you were thinking that could be why not.
