We are a kindergarten. We want to create an admission registration form integrated with booking/ appointment calendar, for customers who did not visit our facility before. Thus, when they register through our website, we would like to ask them if they have visited already (conditional logic in the form); if they say “yes”, then they proceed with standard registration. However, if they say they have not visited before, a booking calendar needs to open and they can select visit date. Is this possible?

Further, which booking/ appointment plugin is compatible with this form. Our visit dates are pre-defined, so customers should only be able to select from pre-defined dates (and predefined time-slots) until slots are fully booked.

    Hello,

Yes, this can be achieved. Using conditional logic (field rules), you can have certain fields show up only if the condition is met (in your case, showing the booking fields only if the answer is “No”).

At this link you can see a demo form that might suit your needs: https://app.captainform.com/form-777886/

The pre-defined dates and time slots can be limited through the following functionality in order for the next user to see which dates are still available.

If you need further assistance with setting up your form, or if you have more questions, feel free to contact us on live chat or at [email protected].

    Best regards,
    The CaptainForm Team
