Interactive questionnaire form that displays posts results

Hello,

I’m trying to build a questionnaire for a client who runs a video course website.

On the homepage, there will be a multi-step form which contains a questionnaire and checkbox fields for each row of questions. These checkbox fields will need to tie in with the tags that are added to a blog post. Some of the fields will not need to tie in with a tag and are required for data collection such as name and email address. When the form is submitted, it should either display the post results there and then or redirect to a new page (similar to a search engine result).
