Intake Forms

I’m looking for a way to have my clients fill out intake forms that will auto-update their customer profiles within the Salon Booking System. The types of forms I’d like would be a General form (allergies, goals, etc) to specific forms regarding specialty needs and waivers. There are 16k or more form plugins, but I don’t think any of them will integrate with this plugin.

I could build one within Mailchimp but it won’t let me add custom fields. I’d love to use Google Forms, but I don’t know how to integrate that with the customers/clients within this plugin

Any help would be appreciated.
