    Don’t see it happening here. Make sure you’re running the proper version of TinyMCE Advanced: 4.1.1 for WordPress 4.0. If that is right, try deleting the browser cache and disabling other plugins.

    Actually this is caused by the hosting company adding HTML in all output from php files. Try asking them to stop, or add

    define('SCRIPT_DEBUG', true);

    near the top of wp-config.php.
