Insert the form in a page

Hello
thank you for your useful plugin I downloaded and installed both the old version “Questions” and the new one and created an application form today . i tried to insert the form into a WP page by inserting the short cut [form id=42] which I copied but the page shows only the page titel not the form. I tried the same in an WP article and it worked but i need my form in a page so I wonder if you could help me with that. I need to change the text on the buttons and other instructions from English to Swedish which was supported in the previous version I was hoping you could help me with that as well
