I have tested my site on Why No Padlock everything is green except below error,

Insecure <form> call.
Found on line # 380 in file: https://www.sanjaypuri.com/index.html

but i don’t have index.html file.

Could you please suggest me.

Have exactly the same issue and no green padlock…

Insecure <form> call.
Found on line # 217 in file: lifeinthefastlane.com/index.html
(Note: Chrome will show a security error for any secure page with an insecure <form> call on the page)

Would love to know if there is a fix for this
