Input form not displaying with recent update.

All of my products, with a recent update, are no longer displaying the text input form on the product pages. I have spent a few hours rechecking everything within the Stock-Alert settings and everything is correct and remains as it has been. I can load a page and see it for a second before it disappears. I’ve tried disabling other plugins as well and nothing changes. Any help is greatly appreciated as it’s been a great addition to our website and we’d love to keep that function for our customers.
