Input Data Not Attached To Order

I just did a test with this product and filled out the form, but when I submitted the order, the order details are not there. It doesn’t show up on the checkout page, in the email, or in the woocoommerce backend. So I can’t see what options a customer chooses. Is there something else I need to do to make it work?
