    Hi arkrausehardie,

    We looked at the form that you provided and it looks like that there is something wrong in the shortcode that you used on the page.
    It should look like this:
    [email-download download_id=”1″ contact_form_id=”4″]
    where download_id is the id of the file which you take from the WP Download Monitor plugin edit page.
    Please confirm that.
