Information not being recorded on Form

Hi there.
I have a problem with the plugin on my site: https://www.surreygreenlanetours.co.uk/index.php/book-online-now
The calendar is working fine and I have already taken bookings and accepted payments, however the information people are entering in to the form is not being recorded and passed to me either on the WP tab (see image on tinypic below) or in the confirmation email I’m receiving
Anything I can do to fix this?
