Images not appearing according to size

I’m having problems posting images on my site. When I save it, it’s the normal size, 620×400 but when I upload it to the site and preview it, it suddenly becomes 300×199. And when I try to resize it to the standard size it starts looking very grainy. I’ve never had this problem before. How can I fix it?
