Image Title Being Inserted Even With Checkbox Unselected

Thanks for reporting this, this was a bug. I have fixed this in the development branch and this will be taken care of in the next version.

I guess no one cared about this yet because having the image title was often a good idea. Any reason why you specifically did not want the title tag?

Can you manually do the change for the time being?

Inside the plugin folder open admin/iaff_image-attributes-from-filename-do.php

and change this line: https://github.com/arunbasillal/Auto-Image-Attributes-From-Filename-With-Bulk-Updater/blob/master/admin/iaff_image-attributes-from-filename-do.php#L425

to: if ( ! ( isset( $settings['image_title_to_html'] ) && boolval( $settings['image_title_to_html'] ) ) ) {
