Image not showing up in Multi-Step Donation Form

Hi there,
I’m setting up a donation form and when I enable the multi-step form, the image does not appear. Upon looking through other threads it may have to do with our Image Optimization plugin, but I have tried to exclude:
div.give-embed-form
div.give-section.introduction
body.give-form-templates
iframe.give_embed_form
from the optimizer and I’m still not able to see the image on our form.
Any ideas as to what else this could be?
