image in uploads doesn’t get added to media library

i have an image
https://letartliveon.com/wp-content/uploads/2015/04/IMG_0088.jpg
used as featured image on
https://letartliveon.com/lalo-president-also-hard-at-work-on-new-book/
but when i try to add it to another post, it doesn’t appear.
i’ve run fix media library, selecting weak refs/add to Media Library,
log shows hundreds of errors fixed, with no processing errors, but the image doesn’t get added:-\
wtf?
