There is nothing in Forminator that would duplicate uploaded images “on purpose” and we didn’t have any reports like that so far. I just tried on my own setup too and couldn’t replicate such issue.
In most unexpected issues of that kind are a matter of some sort of a conflict on site and I suspect this may be the case here. It’s quite possible, actually, that there was no such conflict and it only arose after some update (of Forminator and/or some other plugin on site) but this would need to be checked.
That being said, a full conflict test would be an essential troubleshooting step here:
– disabling (temporarily) all the plugins other than Forminator
– switching theme to e.g. Twenty Twenty-Three (also temporarily)
– then testing the form
If the issue is not happening anymore, enable your original theme back and check again. If still fine, start enabling plugins back one-by-one, after each one checking for the issue. Once it starts to happen, the last enabled plugin right before it would be the most likely cause of the conflict and knowing about it we’d look into it closer.
If the issue is still happening even with default theme and no other plugins active, it would at least rule out the conflict and we’d know that we need to look elsewhere but testing for conflict first is essential
