How are you hiding the name field in the form?

It looks like you have changed the name field to be a phone field. The validation is failing because the name field is required in the settings, but the name field is missing in the form (since you changed it to a phone field).

The Review Forms add-on allows you to add custom fields to the form.

Alternatively, you can try to add custom fields using a code snippet.

Please note: adding custom fields with a code snippet is not supported, but if you want to try and do it on your own, that code snippet should help get you started.

You should never modify the plugin directly as any changes you make will be overwritten every time the plugin is updated.

Instead, use the code snippet example I provided.

To change the title, you can use one of the hooks provided in the Site Reviews documentation:

No. The name field is fixed to a specific column in the custom ratings table. You can’t rename it.
