If 3 stars or lower -> separate ratings template

Hello
I would like to add an additional link if a visitor gives 3 stars or lower. For example I would implement a link to a feedback form, so that the visitor gets the possibility to tell me, what could be improved for the page/post.
Does anybody have an idea how I could implement this?
