Icons not appearing in Edit Post/Page window

I’ve noticed since upgrading the v2.8 on a couple of my clients’ sites that the icons are gone from the post/page editor. The ones in HTML do show up but not in the Visual mode. I deactivated all plugins, cleared my cache and still no change. Why is this happening and what’s the fix?
