I want to add check list of ingredients

I want to add a check list into the product page https://gyro-boss.com/shop/appetizer/spanakopita/ a list of ingredients. Now if the people doesn’t want a ingredient, they can uncheck or place a note and this information will be part of the order. Similar to https://www.specialtys.com/Products.aspx?MenuId=228.
