I need 2 shipping rates?

How do I get a shipping cost added? I see it is saying Shipping and then it is saying configure shipping I push on it and nothing happens? I need to add a $12.00 flat fee for items up to $49.00 and give free shipping on all orders over $49.00? please tell me how to do this if anyone knows and I thank you in advance.
