There are two ways of doing that, one is to enforce a specific format. You can do it using a snippet like this one: https://gist.github.com/patrickfreitasdev/7a15a0b9b8b479b11c2e64771135d70b (you’ll need to edit it to adjust the code to your needs and place the .php file in wp-content/mu-plugins)

In this case the key things to edit are:
– the masks on lines 34, 42, 44
– on line 52 the custom class that needs to be added to the field in question
– also if needed uncomment the lines 55-58
