You can do this via the post editor. Just add an image above the contact form’s shortcode.

Thank you so much for your reply. Yes, that’s what I had done. The problem I encountered was that the text of the form flows next to the picture rather than underneath the picture. So it looks messy. I tried adding line and paragraph breaks in the coding but nothing worked.
