I badly need help with what could be very simple, yet its elusive

So far Ive bought the eStore plugin from tips and tricks, seen the form that PAYPAL gives you through the button format, and bought the Marketplace plugin from wpmudev… but I CANNOT for the love of god know how to build this simple form.

Its found everywhere … most form plugins like cforms dont have a checkout option…

Ive tried dealing with WP quote generator, but it wont let checkout …

Ofcourse Im a PHP newbie, but no criminal. Please someone help me in getting a form like this setup on my website, Ive tried looking on graphic river, but cannot afford to buy and not use something… help please………..!!!! Guys on freelancer wont respond to my emails after they underbid on my project.
